Appointment Reminder

BEFORE YOUR APPOINTMENT
Please arrive 10 minutes prior to your scheduled time and log into your patient portal to review and
complete all required online consent forms before your visit.

CANCELLATION POLICY
We require a minimum of 48 hours’ notice for all cancellations. Late cancellations are subject to the
following fees:

Standard appointment $50.00
Filler appointment $100.00
